Overview
We are seeking compassionate and dedicated Licensed Vocational Nurses (LVNs) to provide high-quality care and support to adults with complex medical and mental health needs, including individuals experiencing homelessness and substance use disorders.
These contract positions offer the opportunity to make a meaningful impact while working within a collaborative, trauma-informed, and harm-reduction-focused environment. Multiple schedules are available at different program locations throughout Los Angeles.

LVN – South Los Angeles (90037)
- Schedule: Monday & Tuesday
- Hours: 12-hour shifts
- Status: Contract (Month-to-Month)
LVN – Los Angeles (90002)
- Schedule: Saturday & Sunday
- Hours: 8-hour shifts
- Status: Contract (Month-to-Month)
Key Responsibilities
Direct Nursing Care & Coordination
- Administer medications, injections, and other prescribed treatments.
- Monitor client health status and report significant changes to the appropriate providers.
- Coordinate primary care and specialty medical appointments.
- Maintain accurate medication logs and treatment records.
- Collaborate with pharmacies to ensure timely medication delivery.
- Provide health education and support to promote medication adherence and wellness.
Collaborative, Integrated Care
- Work closely with an interdisciplinary team to provide comprehensive, client-centered care.
- Support clients with severe mental illness (SMI), co-occurring disorders, and complex medical needs.
- Assist with crisis intervention and coordinate emergency services when appropriate.
- Participate in team meetings, case conferences, and supervision sessions.
Documentation & Compliance
- Complete timely and accurate documentation in the Electronic Health Record (EHR) system.
- Maintain compliance with HIPAA and all applicable healthcare regulations.
- Assist clients with navigating healthcare services and accessing available benefits.
- Ensure all documentation meets organizational, county, state, and funding-source requirements.
Qualifications
Education & Experience
- Experience in nursing, behavioral health, social services, or a related healthcare setting.
- One (1) to three (3) years of experience working with individuals experiencing severe mental illness, substance use disorders, homelessness, or other complex populations preferred.
Skills & Abilities
- Strong clinical assessment and patient care skills.
- Ability to maintain professionalism and composure in challenging situations.
- Excellent communication and interpersonal skills.
- Experience using Electronic Health Records (EHRs) and healthcare technology.
- Understanding of trauma-informed care and harm-reduction principles preferred.
Licenses & Certifications
- Current California Licensed Vocational Nurse (LVN) license (required).
- Current CPR certification (preferred).
- Valid California Driver’s License with a clean driving record and insurability (required).
- Live Scan clearance (required).
- COVID-19 vaccination and booster (required).
