Overview:
Are you an adaptable, compassionate nurse dedicated to helping vulnerable seniors maintain their independence and health? We are seeking a proactive Licensed Vocational Nurse (LVN) to join our clinical operations team. In this vital clinic-facing role, you will work closely with our attending Providers and Clinical Nurse Manager to deliver high-quality patient care in accordance with your professional scope of practice. This position balances specialized, hands-on clinical duties, such as wound care, point-of-care (POC) testing, and medication administration, with dedicated care coordination support. Serving as a crucial clinical liaison between our senior participants, their family caregivers, and the interdisciplinary medical team, you will play an essential role in unlocking the full potential of the seniors we serve.


What you will be doing:
Clinical Nursing Care

  • Direct Patient Treatment: Perform and document ordered wound care, clinical procedures, and therapies in the clinic setting as directed by the Primary Care Provider (PCP).

  • Medication Administration: Safely administer ordered medications to participants while in the Center or Clinic, reinforcing vital patient education regarding their drug regimens.

  • Medication Reconciliation: Perform thorough medication reviews and reconciliations during "Welbe Welcome Day" visits for new participants, and conduct monthly inventories of clinical medication systems (such as the ATM machine, if applicable).

  • Specialty Orders: Process specialty pharmacy medication orders, addressing participant inquiries and escalating clinical issues to the PCP as needed.

  • Transition Coordination: Partner with providers to manage same-day medication order changes stemming from outpatient procedures and external medical appointments.

  • High-Risk Monitoring: Conduct and document high-risk participant touchpoints, including specialized vitals monitoring and direct medication reminders.

  • Proactive Follow-ups: Perform quality touchpoints for Yellow/Green risk-stratified participants as outlined by the RN Case Manager, escalating changing needs to the PCP or RN promptly.

  • Perioperative Support: Complete and document pre- and post-operative clinical outreach, reviewing specialist instructions and managing follow-up patient care.

  • Condition Assessment: Remain alert to physical, cognitive, or environmental changes reported by caregivers or staff, communicating changes immediately to the clinical team to maintain continuity of care.

  • Other dutiies as assigned.


What you must have:

  • Education: Graduate of an accredited School of Nursing (Associate's Degree or community college equivalent).

  • Licensure: Active, unencumbered Licensed Vocational Nurse (LVN) license in the state of California.

  • Experience Baseline: Minimum of one (1) year of professional nursing experience in a clinical setting working directly with a frail or elderly population.

  • Clinical Knowledge: Deep clinical knowledge and procedural skills necessary to treat frail, elderly individuals and safely manage complex medical situations.

  • Mobility: Must possess a reliable means of transportation to travel to and from the clinic-facing work location.

Salary/Rate Range: $35.00 -$39.00/hour depending on years of experience

Location and Work Type

  • Work Type: Full-Time Contract Position (40 hours per week).

  • Location: On-site clinical environment in Stockton, CA.

  • Schedule: Standard daytime hours: Monday through Friday, 8:00 AM – 4:30 PM (No overnight on-call required, though occasional scheduling flexibility may be needed to support business operations).
