Working under the direct guidance of a licensed Physical Therapist, you will play a crucial role in delivering restorative and rehabilitative physical therapy services to senior participants at our day health center and within their homes. This position balances hands-on clinical therapy—such as leading therapeutic exercises, balance training, and gait rehabilitation—with administrative support to keep our department running smoothly. Duties & Responsibilities
Therapeutic Care & Intervention Delivery

Implement Treatment Plans: Administer individualized physical therapy treatments and exercise programs exactly as established by the supervising Physical Therapist.

Clinical Documentation: Accurately deliver and document interventions focused on muscle re-education, balance, endurance, and gait training to improve participants' independence in Activities of Daily Living (ADLs).

Assessment Assistance: Assist the Physical Therapist in obtaining vital signs, measuring range of motion, and conducting strength testing.

Emergency & End-of-Life Care: Remain highly alert to changes in a participant's functional status, reporting modifications directly to the PT and interdisciplinary team (IDT). Participate in supervised end-of-life care coordination.

Administrative Operations: Manage patient treatment scheduling, coordinate clinical visits, and handle rehabilitation billing processes.

Equipment & Logistical Upkeep: Track and manage routine maintenance for therapeutic equipment, monitor daily clinical logs (such as temperature logs), and maintain inventory of essential therapy supplies.

Caregiver Training: Instruct staff, participants, and family caregivers on the safe use and maintenance of therapeutic equipment.

Other duties as assigned.
Required Qualifications
Education, Licensure & Credentials

Education: Associate's Degree in Physical Therapy Assistant from an accredited program (qualifying degrees earned outside of the US must be evaluated to the US equivalent).

Licensure: Valid, unrestricted California Physical Therapy Assistant License.

Work Authorization: Legally authorized to practice in the state of California.
Experience & Skills

Clinical Focus: Minimum of two (2) years of Physical Therapist Assistant experience in a clinical setting working directly with a frail or elderly population.

Mobility: Must possess reliable personal transportation to complete required clinic-based and occasional field-based mobile therapy visits.

Core Skills: Comprehensive clinical knowledge of geriatric conditions, strong organizational skills, and the ability to process billing and scheduling data accurately.

Work Type: Full-Time Contract Position (40 hours per week).

Work Setting: Primarily on-site clinical day health center environment with occasional field-based mobile visits (SNFs, ALFs, or homes) based on participant needs. Located in Stockton, CA.

Schedule: Monday through Friday, 8:00 AM – 4:30 PM

Pay: $45-48/HR depending on years of experience.
