Role Summary
Under the direction and supervision of the Supervisor or Manager, the Caregiver 2 provides compassionate, high-quality personal care services to patients receiving home care and/or day center services. This role is essential in supporting patients’ independence, dignity, and overall well-being.
In addition to direct care responsibilities, the Caregiver 2 serves as a mentor and trainer to other caregivers, helping ensure consistency, safety, and excellence in service delivery. This position is ideal for an experienced caregiver who values teamwork, leadership, and making a meaningful difference in the lives of frail and elderly individuals.
Duties & Responsibilities

Provide personal care services to patients in day center settings

Provide personal care and escort assistance in accordance with the individual care plan, including grooming, dressing, peri-care, bathing, toileting, housekeeping, meal preparation and feeding, and laundry

Observe, document, and report changes in patient condition to the care team in a timely and professional manner

Demonstrate safe care practices by utilizing proper body mechanics, adhering to universal precautions, and maintaining a safe environment

Interact respectfully and professionally with patients, families, and the care team, encouraging engagement in conversation and activities when appropriate

Complete routine and repetitive care tasks accurately and consistently

Assist with onboarding and ongoing training of Caregiver 1 and Caregiver 2 staff using established manuals and formal training materials

Actively participate in departmental meetings and family meetings as appropriate

Promote patient safety by properly using and educating others on lifting and safety equipment

Support a collaborative, respectful team environment

Perform other related duties as assigned
Required Qualifications
-
Minimum of one (1) year of experience working with frail or elderly individuals
-
Home Health Aide (HHA) or Certified Nursing Assistant (CNA) certification required
-
Current Basic First Aid and CPR certification
-
Ability to work independently and collaboratively as part of a team
-
Flexibility and a positive attitude when working across multiple care settings
-
Knowledge and proper use of safety equipment for lifting; ability to coach others in safe techniques in a supportive manner
-
Physical ability to stoop, reach, stand, walk, push, pull, lift, grasp, speak, hear, and perform repetitive wrist and hand motions

Visual acuity and ability to lift/move weight within personal physical capacity, using appropriate lifting devices or requesting assistance when needed

Basic English reading and writing skills required

Basic computer skills, including the ability to use or learn multiple applications such as email and electronic health records
Desired Qualifications

Familiarity with coordinated care models serving elderly populations

Bilingual

Driver license
Location & Work Type
On-site role providing services in the day center settings. May require flexibility to work across multiple care locations.

Full Time Monday through Friday 8 AM - 4:30 PM
