Home Health Registered Nurse (RN)
Location: Hermiston, OR
Schedule: Day Shift | Monday–Friday, 8:00 AM – 4:30 PM | Weekend Rotation

As a Home Health RN, you will provide personalized nursing care, helping patients recover from illness, manage chronic conditions, and maintain independence in the comfort of their own environment.
What You’ll Do
• Provide skilled nursing care to patients in their homes, including private residences, assisted living, and adult foster homes
• Perform comprehensive patient assessments and develop individualized care plans
• Monitor patient progress and adjust care plans to support optimal outcomes
• Educate patients and families on disease management, medications, and recovery
• Collaborate with physicians and interdisciplinary teams to coordinate care
• Maintain accurate, timely documentation of assessments, treatments, and outcomes
• Advocate for patients and support their safety, comfort, and well-being
Qualifications
Required
• Current RN license in Oregon
• BLS certification
• Minimum 1 year RN experience in acute care
• Valid Oregon driver’s license, reliable vehicle, and current auto insurance
Preferred
• Home Health, Community Health, or Critical Care experience
• BSN
Schedule
• Monday – Friday, 8:00 AM – 4:30 PM
• Rotating weekend coverage shared among clinicians
• Maximum 40 hours per week
