Overview:
Our client is looking for a Registered Nurse to joint their amazing  team. Responsibilities include conducting health assessments, developing care plans, coordinating care with interdisciplinary team members, and providing direct nursing care. The nurse ensures that patients receive high-quality, person-centered care that supports their health, safety, and independence.

• Setting: Out Patient Clinic

• Population: Geriatric population

What you will be doing:

• Conduct thorough patient assessments to determine health status and care needs.

• Monitor and document patient progress, adjusting care plans as necessary.

• Develop and implement individualized care plans in collaboration with the team.

• Provide direct nursing care, including medication administration, wound care, and other treatments.

• Administer medications, treatments, and other nursing interventions as prescribed.

• Educate patients and their families about health conditions, medications, and self-care strategies.

• Coordinate care with physicians, therapists, social workers, and other healthcare providers.

• patients in interdisciplinary team meetings and case conferences.

• Facilitate communication among the interdisciplinary team to ensure comprehensive care.

• Maintain detailed and confidential patient records.

• Ensure compliance with healthcare regulations and standards.

• Provide compassionate, patient-centered care in accordance with nursing ethics and standards.

• Other duties as assigned

What you must have:

• 2 years of experience

• Current Registered Nurse (RN) license.

• Associate or Bachelor's degree in Nursing (BSN preferred).

• BLS from the American Heart Association

• Drivers license

• Experience in geriatrics, home health, or a similar setting preferred.
