Overview:
Our client is seeking a compassionate and dedicated Social Worker to provide direct care management services to participants in their program. The Social Worker will play a crucial role in delivering social services support, including assessments, counseling, case management, and referrals. - Setting: Outpatient Clinic working with the geriatric population, with occasional home visitations as needed.
What you will be doing:
- Participate in the initial assessments, care planning, and periodic re-assessments (minimally every 6 months) of participants in alignment with industry and departmental standards.
- Document participant changes accurately and in a timely manner, in accordance with medical record standards.
- Maintain open and effective communication with other team members regarding participant status and needs.
- Oversee the coordination of 24-hour care delivery, providing individual and family counseling when necessary.
- Lead group counseling sessions and activities designed to support participants' emotional and social well-being.
- Refer participants and families to relevant community agencies, acting as a liaison and advocate to ensure participants' needs are met.
- Foster positive relationships among staff, promoting team building and a collaborative work environment.
- Consult with staff members regarding the impact of social, emotional, and cultural factors on health and care.
- Contribute to program and policy development in social work and actively participate in quality improvement processes.
- Conduct home visits to participants’ private residences.
- Attend and actively participate in interdisciplinary team meetings to contribute to participant care planning.
- Perform other duties and projects as assigned.

What you must have:
- Master’s degree in Social Work from an accredited school of social work.
- Current state licensure and membership in the Academy of Certified Social Workers (ACSW) or other NASW-recognized certification is preferred.
- At least two (2) years of experience working on an interdisciplinary team in a hospital, nursing home, or community-based setting is preferred.
- A minimum of one (1) year of experience working with a frail or elderly population is preferred. If not, training will be provided upon hire.
- Must have reliable transportation, a valid driver’s license, and the minimum required state auto insurance.
- Medical clearance for communicable diseases and up-to-date immunizations before engaging in direct participant contact.
- Successful completion of a criminal background check, including federal and state Medicare/Medicaid exclusion lists, criminal history, education verification, license verification, reference check, and drug screening.
Nice to have:
- Experience with individual, family, and group counseling.
- Strong interpersonal and conflict resolution skills.
- Ability to understand and address the psychosocial issues faced by frail and chronically ill individuals and their caregivers.
- Strong analytical, problem-solving, and organizational skills.
- Familiarity with public speaking and delivering training to various group sizes.
- Basic computer proficiency and familiarity with electronic communication tools.
