Overview:
The Registered Dietitian plays a vital role in promoting wellness and improving patient health outcomes by delivering comprehensive nutritional assessments, counseling, and care planning. This position supports patients across a variety of care settings, ensuring their dietary needs are met with compassion, expertise, and evidence-based practice. As an essential member of the team, the Registered Dietitian collaborates closely with clinical and social care professionals to develop personalized care plans, monitor nutritional status, and provide high-quality education to patients, families, caregivers, and staff. This role offers the opportunity to make a meaningful impact through person-centered nutrition support and continuous quality improvement.


What you will be doing:

  • Provide high-quality nutritional care and actively contribute as a member of the team.
  • Conduct initial and ongoing comprehensive nutritional assessments for all patients as required by program procedures and regulations.
  • Collaborate with the team to develop, implement, and revise individualized care plans.
  • Offer nutritional counseling and education to patients, caregivers, and families to support health, wellness, and disease management.
  • Facilitate nutrition-related interventions, including home-delivered meals and supplements.
  • Monitor changes in weight, medical conditions, medications, hydration, swallowing ability, mood, skin integrity, and overall intake, reporting findings and recommendations to the team.
  • Provide guidance to team members, contractors, and center staff regarding patients’ nutritional needs and related health issues.
  • Incorporate patients’ cultural and ethnic dietary preferences into care planning and nutritional services.
  • Perform duties of other team members or the Food Service Supervisor role as needed based on licensure and competencies.
  • Participate in quality improvement initiatives, team meetings, family meetings, staff training, and orientation programs.
  • Maintain compliance with safety guidelines, infection control protocols, and organizational policies.
  • Ensure documentation is clear, concise, comprehensive, and reflective of high-quality care.
  • Protect patient privacy and maintain confidentiality of all sensitive information.
  • Perform other duties as required.

What you must have:
  • Bachelor’s degree in food, nutrition, or dietetics from an accredited institution.
  • Passed the Commission on Dietetic Registration (CDR) exam.
  • Minimum of one year of experience working with the elderly population
    (training may be provided upon hire if needed).
  • Reliable transportation, valid driver’s license, and required auto insurance.
  • Medical clearance for communicable diseases and up-to-date immunizations before providing direct patient care.
  • Successful completion of a comprehensive background check, which may include Medicare/Medicaid exclusion verification, criminal history, education verification, license verification, references, and drug screening.
  • Strong computer literacy, including proficiency with Microsoft Outlook, Word, Excel, and PowerPoint.
  • Ability to effectively communicate in writing and verbally.

Nice to have:
  • Two or more years of clinical nutrition experience.
  • Serve Safe Certification.
  • Experience providing medical nutrition therapy for chronic conditions common among geriatric patients (e.g., diabetes, heart disease).
  • Familiarity with managed care environments, quality indicators, and cost-effective service delivery.
  • Strong understanding of home- and community-based nutritional services.
  • Ability to plan creatively, problem-solve, and think innovatively when addressing patient needs.
  • Experience with electronic health records and management information systems.
  • Commitment to person-centered care, patient advocacy, and collaborative team-based decision making.
Physical Requirements
  • Ability to lift and carry up to 30 pounds using proper body mechanics.
  • Manual dexterity and hand–eye coordination for operating department equipment.
  • Normal or corrected vision and hearing, with effective phone communication skills.
  • Location & Work Type: On-site role within a clinical and center-based care setting.
