What you will be doing:
Clinical Practice & Case Management
- Empowering Care Coordination: Utilize independent judgment and discretion to manage a patient panel, ensuring comprehensive and continuous care.
- Comprehensive Assessments: Conduct initial and ongoing psychosocial assessments to identify risks, evaluate needs, and design personalized care plans tailored to each patient's goals.
- Counseling & Advocacy: Provide individual counseling, facilitate family conferences, and run patient and caregiver support groups or workshops.
- Community Resources & Transitions: Connect patients to vital community organizations (e.g., residential care facilities, skilled nursing, social services) and manage thoughtful discharge planning when needed.
- Peer Support: Review and verify assessment findings completed by non-MSW team members when appropriate.
Team Collaboration & Quality Excellence
- Patient & Family Liaison: Serve as the essential bridge connecting patients, family caregivers, the care team, and community partners.
- Interdepartmental Synergy: Partner closely with internal departments and healthcare agencies to deliver holistic, 5-star service based on compassion, excellence, resourcefulness, and inclusivity.
- Active Team Engagement: Participate in team meetings to advocate for patient priorities, build consensus, and drive unified care strategies.
- Compliance & Record-Keeping: Complete all clinical documentation and updates accurately, timely, and in full compliance with ethical, legal, safety, and HIPAA confidentiality standards.
What you must have:
Education & Credentialing
- Licensed Specialty: Active Licensed Clinical Social Worker (LCSW) license required.
- Degree: Master’s Degree in Social Work (MSW) from an accredited college or university. (Candidates with a Bachelor’s Degree in Social Work combined with required language skills and geriatric experience may also be considered).
- Licensure: Valid Social Worker license in the State of California.
- Certification: Active MOCA Certification required.
Experience & Technical Skills
- Geriatric Care Expertise: Minimum of one (1) year of experience working with a frail or elderly population.
- EHR Proficiency: Minimum of one (1) year of hands-on experience utilizing EPIC.
- Core Competencies: Strong organizational skills, clinical assessment capability, and proficiency in standard software applications (Microsoft Office).
- Communication & Resilience: Excellent written and verbal communication skills with the ability to maintain composure, adapt flexibly, and perform effectively under shifting priorities.
Nice to have:

Language Abilities: Chinese language skills (Cantonese or Mandarin) strongly preferred; general bilingual fluency is a significant asset.
- Advanced Experience: Minimum of one (1) year of post-MSW experience, healthcare case management experience, or background in a senior care setting.
- Collaborative Mindset: Strong track record of interdisciplinary collaboration, care coordination, and managing a full-time caseload in a fast-paced environment.
Salary/Rate Range: $45-$59/HR

- Work Type: Full-Time Position (Contingent/Contract).
- Work Setting: Hybrid geriatric care environment, encompassing professional office settings, ambulatory clinics, patient homes, and residential care facilities.
