Licensed Practical Nurse (LPN)

Schedule

• Four 10-hour shifts per week

• 6:00 AM - 4:30 PM

• Every other weekend

• Holidays as needed
  
  Rate: 37/hr local and 41/hr travel

Position Summary

We are seeking an experienced and dependable Licensed Practical Nurse (LPN) to provide high-quality nursing care in a collaborative healthcare environment. The LPN will provide direct patient care, administer medications and treatments, monitor patient conditions, and work closely with Registered Nurses, providers, and other members of the healthcare team.

Key Responsibilities

• Provide direct nursing care in accordance with established policies, procedures, and individual plans of care.

• Administer medications safely and accurately in accordance with medication administration standards and provider orders.

• Obtain and document vital signs and promptly report abnormal findings or changes in patient condition.

• Perform patient assessments within the LPN scope of practice and communicate findings to the Registered Nurse and/or provider.

• Complete point-of-care specimen collection, including phlebotomy, as required.

• Administer vaccines and other IM/SubQ medications as ordered.

• Perform routine treatments, including wound care and other nursing procedures within scope of practice.

• Assist with management of acute and chronic healthcare needs.

• Recognize changes in patient condition and respond appropriately to urgent or emergency situations in accordance with BLS standards.

• Provide patient education regarding medications, treatments, follow-up care, and health maintenance.

• Maintain accurate, timely, and complete documentation of patient care and interventions within the electronic health record.

• Communicate effectively with physicians, advanced practice providers, Registered Nurses, and other members of the healthcare team.

• Maintain compliance with infection-control, patient-safety, confidentiality, and regulatory requirements.

• Perform additional duties as assigned.

Qualifications

• Current and unrestricted LPN/LVN license in the applicable state.

• Current BLS certification.

• Previous clinical nursing experience preferred.
