Licensed Practical Nurse (LPN)

Position Overview

We are seeking an experienced and dependable Licensed Practical Nurse (LPN) to join our healthcare team. This position is part of the LPN pool and plays an essential role in daily Health Services Unit operations, including direct patient care, medication administration and management, health screenings, treatments, laboratory support, and emergency response.

The LPN will work closely with Registered Nurses and other healthcare professionals to ensure patients receive timely, safe, and appropriate care.

Schedule

• Monday-Friday: 8:00 AM-4:30 PM

• Weekend Rotation: Every third weekend, 10:00 AM-6:30 PM

• Holiday Rotation: Every third holiday, 10:00 AM-6:30 PM

• Position participates in the established LPN weekend and holiday rotation.

Key Responsibilities

Direct Patient Care & Medication Administration

• Provide direct patient care in accordance with established policies, procedures, and scope of practice.

• Administer medications during scheduled nurse medication/pill-pass activities.

• Administer facility injections, including MAT/MOUD medications, biologics, insulin, and other prescribed injectable medications.

• Assist patients with blood glucose checks and nurse-controlled insulin administration.

• Administer and support specialized medication treatments, including warfarin and hepatitis treatments.

• Perform wound care and other ordered treatments.

• Conduct restricted housing rounds and provide appropriate nursing care and follow-up.

• Perform routine testing, screenings, and delegated nursing treatments and tasks.

• Assist with diabetic foot care, perineal care, injections, and other patient-care needs within the LPN scope of practice.

Health Screenings & Laboratory Support

• Conduct annual health maintenance screenings and other required patient screenings.

• Document screening results and communicate concerns or abnormal findings to the appropriate healthcare provider.

• Cross-train in laboratory functions and assist with routine laboratory-related duties as needed.

• Support RNs with delegated clinical tasks and treatments.

Medication Management

• Support daily medication-room operations and ensure adequate medication supplies are maintained for institutional needs.

• Order and request medications according to established procedures.

• Receive, store, organize, and maintain medications in accordance with applicable policies and standards.

• Administer medications accurately and safely.

• Help ensure continuity of medication therapy and promptly address medication supply issues that could result in patients missing essential medications.

• Maintain accurate medication documentation and records.

Emergency Response

• Respond to medical emergencies alongside the emergency RN.

• Assist with transporting and managing the emergency cart, medications, AED, and other emergency equipment.

• Obtain and document vital signs during emergency situations.
