RN Case Manager

As the RN Case Manager, you will coordinate comprehensive patient care plans while serving as the bridge between patients, families, physicians, post-acute providers, and community resources. You will help ensure patients receive the right care at the right time while supporting safe, efficient transitions of care.

This role is ideal for nurses who are highly organized, compassionate communicators, and passionate about improving patient outcomes.

Key Responsibilities

  • Perform comprehensive patient assessments including clinical, psychosocial, environmental, and discharge planning needs
  • Develop and implement individualized care plans in collaboration with interdisciplinary teams
  • Coordinate transitions between acute care, rehabilitation, home health, long-term care, and community services
  • Facilitate discharge planning and identify barriers to safe discharge
  • Collaborate with physicians, therapists, social workers, and healthcare teams to ensure continuity of care
  • Advocate for patients and families while navigating complex healthcare systems
  • Assist with insurance authorizations and utilization review processes
  • Educate patients and families regarding treatment plans, resources, and post-discharge care
  • Connect patients with community programs and support services
  • Maintain accurate documentation and case management records
  • Participate in interdisciplinary rounds and care conferences

Qualifications

Required:

  • Active Oregon RN License
  • Previous acute care, home health, or skilled care assessment experience
  • Graduate of an accredited nursing or healthcare-related program

Preferred:

  • Previous Case Management and/or Discharge Planning experience
  • BSN or MSN
  • Bilingual English/Spanish skills
  • Experience coordinating post-acute community resources
  • Experience working with culturally diverse populations
