Overview:
As the RN Case Manager, you will coordinate comprehensive patient care plans while serving as the bridge between patients, families, physicians, post-acute providers, and community resources. You will help ensure patients receive the right care at the right time while supporting safe, efficient transitions of care. This role is ideal for nurses who are highly organized, compassionate communicators, and passionate about improving patient outcomes.
What you will be doing:

Perform comprehensive patient assessments including clinical, psychosocial, environmental, and discharge planning needs

Develop and implement individualized care plans in collaboration with interdisciplinary teams

Coordinate transitions between acute care, rehabilitation, home health, long-term care, and community services

Facilitate discharge planning and identify barriers to safe discharge

Collaborate with physicians, therapists, social workers, and healthcare teams to ensure continuity of care

Advocate for patients and families while navigating complex healthcare systems

Assist with insurance authorizations and utilization review processes

Educate patients and families regarding treatment plans, resources, and post-discharge care

Connect patients with community programs and support services

Maintain accurate documentation and case management records

Participate in interdisciplinary rounds and care conferences
What you must have:

Strong critical thinking and care coordination skills

Compassionate, patient-centered communication style

Ability to manage multiple priorities in a fast-paced environment

Experience working collaboratively across healthcare disciplines

Passion for advocacy, education, and improving patient outcomes

Active Oregon RN License
-
Previous acute care, home health, or skilled care assessment experience

Graduate of an accredited nursing or healthcare-related program
Preferred

Previous Case Management and/or Discharge Planning experience

BSN or MSN
-
Bilingual English/Spanish skills
-
Experience coordinating post-acute community resources
-
Experience working with culturally diverse populations
