Role Summary
The Director of Outreach & Engagement plays a pivotal role in driving organizational growth by strengthening community integration and ensuring accurate, supportive enrollment of patients into a comprehensive, all-inclusive senior care program. Reporting to the Executive Director, this leader builds strategic relationships with healthcare providers, businesses, and community groups, while guiding a seamless intake and enrollment experience for patients and their families. This role champions a people-first philosophy and ensures all outreach and enrollment activities align with organizational values, regulatory requirements, and best practices.
Duties

Develop and implement the Outreach and Engagement Plan and related activities.

Ensure the organization’s mission, values, and promise are fully integrated into outreach strategies.

Use demographic data, community needs, competitive insights, historical results, and national standards to guide effective outreach, intake, and enrollment processes.

Set referral targets for the team in alignment with net growth goals established with the Executive Director.

Build a detailed schedule of weekly, monthly, and annual outreach events and initiatives.

Engage a wide range of targeted organizations, groups, and individuals through proactive outreach efforts.

Oversee successful patient intake and enrollment through well-developed departmental policies, procedures, and compliance practices.

Maintain a person-centered approach that enhances every encounter with potential enrollees and referral sources.

Promote a people-first culture that consistently strives to exceed expectations.

Ensure all regulatory standards related to outreach, intake, and enrollment are met.

Communicate regularly with patients, families/caregivers, staff, and other stakeholders.

Meet and aim to exceed census growth and enrollment targets.

Provide individual and group presentations to potential referral sources.

Maintain strong, ongoing relationships with existing referral partners.

Develop and cultivate new referral sources to expand reach and community engagement.

Ensure accountability for all outreach and intake systems within the department.

Maintain detailed referral tracking systems, mailing lists, and marketing records.

Provide regular reports on outreach and engagement activities to the Executive Director.

Lead, supervise, and evaluate departmental staff.

Participate in outreach events, intake activities, open houses, and community marketing events.

Attend required staff meetings and in-service training sessions.

Champion diversity, equity, and inclusion, integrating these principles into departmental culture and practices.

Follow all organizational policies, procedures, and OSHA safety guidelines.

Uphold confidentiality regarding company information, patients, employees, and families.

Follow Standard Precautions and Infection Control procedures.

Maintain a safe working environment.

Support and participate in quality improvement initiatives.

Engage in continuing education and required training.

Maintain professional affiliations and required certifications.

Perform other duties as assigned.
Required Qualifications

Bachelor’s degree.
-
Reliable transportation, valid driver’s license, and state-required liability auto insurance.

Ability to pass all pre-employment screenings, including criminal background check, Medicare/Medicaid exclusion screening, education and license verification, reference check, drug screen, and required immunization verification.

Medically cleared for communicable diseases and up-to-date immunizations prior to direct patient contact.
Desired Qualifications
-
Master’s degree preferred.
-
Professional license and/or certification preferred (e.g., NHA, RN, LSW/MSW, PT, OT, RT, RD).

Experience in community outreach, intake, enrollment, or marketing.
-
Management experience within a human services organization.
-
At least one year of experience working with a frail or elderly population (training will be provided if not already present).
