Primary Care Physician 

We are seeking an experienced and compassionate Primary Care Physician for a 3 month contract assignment with strong potential to extend long term.  You will be providing comprehensive medical care to individuals in a residential healthcare setting. You will be responsible for managing a caseload of approximately 35 to 45 individuals, providing primary care services, conducting medical assessments, participating in interdisciplinary care planning, and supporting the overall health and well being of the population served.

This role offers a collaborative environment with a manageable patient volume, allowing providers to focus on delivering high quality, patient centered care.

Schedule

  • Monday through Friday

  • 8:00 AM to 5:00 PM

  • Participation in weekday on call rotation approximately every 6 weeks (5:00 PM to 8:00 AM)

  • Weekend and holiday on call rotation approximately every 6 weeks

On Call Responsibilities

  • Approximately 95% of on call duties are managed by telephone consultation.

  • Occasional return to the facility may be required for urgent situations.

  • During assigned weekend or holiday call coverage, physician must:

    • Conduct rounds in a 16 bed infirmary providing acute and chronic care services.

    • Evaluate and treat acute illnesses and injuries as needed.

    • Provide telephone consultation and clinical support outside of rounds.

Key Responsibilities

  • Provide primary care services for individuals with acute illnesses, injuries, and follow up care needs.

  • Typically see no more than 10 patients per day.

  • Conduct annual physical examinations and interval medical assessments.

  • Develop and manage treatment plans based on patient needs.

  • Participate in interdisciplinary team meetings and care planning discussions.

  • Review diagnostic results and coordinate appropriate follow up care.

  • Maintain accurate and timely documentation within the electronic health record.

  • Collaborate with nursing staff, specialists, therapists, and other healthcare professionals to ensure continuity of care.

  • Provide clinical guidance during on call coverage periods.

Required Qualifications

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited institution.

  • Current unrestricted Texas medical license.

  • Current DEA registration.

  • Current National Provider Identifier (NPI).

  • Current CPR/BLS certification.

  • Board Certified or Board Eligible in Family Medicine, Internal Medicine, or a related primary care specialty.

  • Clean professional record with no history of disciplinary action, sanctions, or restrictions by any State Medical Board.

Preferred Qualifications

  • Experience caring for individuals with Intellectual and Developmental Disabilities (IDD).

  • Experience working in residential healthcare, state supported living centers, long term care, behavioral health, or similar settings.

  • Familiarity with electronic health records, particularly Cerner.

  • Previous experience working in a State Hospital, State Supported Living Center, or comparable healthcare environment.
