Pay Rate: $35.00 – $37.00 per hour (DOE)


Overview

We are seeking compassionate and dedicated Licensed Vocational Nurses (LVNs) to provide high-quality care and support to adults with complex medical and mental health needs, including individuals experiencing homelessness and substance use disorders.

These contract positions offer the opportunity to make a meaningful impact while working within a collaborative, trauma-informed, and harm-reduction-focused environment. Multiple schedules are available at different program locations throughout Los Angeles.

Available Schedules & Locations

LVN – South Los Angeles (90037)

  • Schedule: Monday & Tuesday
  • Hours: 12-hour shifts
  • Status: Contract (Month-to-Month)

LVN – Los Angeles (90002)

  • Schedule: Saturday & Sunday
  • Hours: 8-hour shifts
  • Status: Contract (Month-to-Month)

Key Responsibilities

Direct Nursing Care & Coordination

  • Administer medications, injections, and other prescribed treatments.
  • Monitor client health status and report significant changes to the appropriate providers.
  • Coordinate primary care and specialty medical appointments.
  • Maintain accurate medication logs and treatment records.
  • Collaborate with pharmacies to ensure timely medication delivery.
  • Provide health education and support to promote medication adherence and wellness.

Collaborative, Integrated Care

  • Work closely with an interdisciplinary team to provide comprehensive, client-centered care.
  • Support clients with severe mental illness (SMI), co-occurring disorders, and complex medical needs.
  • Assist with crisis intervention and coordinate emergency services when appropriate.
  • Participate in team meetings, case conferences, and supervision sessions.

Documentation & Compliance

  • Complete timely and accurate documentation in the Electronic Health Record (EHR) system.
  • Maintain compliance with HIPAA and all applicable healthcare regulations.
  • Assist clients with navigating healthcare services and accessing available benefits.
  • Ensure all documentation meets organizational, county, state, and funding-source requirements.

Qualifications

Education & Experience

  • Experience in nursing, behavioral health, social services, or a related healthcare setting.
  • One (1) to three (3) years of experience working with individuals experiencing severe mental illness, substance use disorders, homelessness, or other complex populations preferred.

Skills & Abilities

  • Strong clinical assessment and patient care skills.
  • Ability to maintain professionalism and composure in challenging situations.
  • Excellent communication and interpersonal skills.
  • Experience using Electronic Health Records (EHRs) and healthcare technology.
  • Understanding of trauma-informed care and harm-reduction principles preferred.

Licenses & Certifications

  • Current California Licensed Vocational Nurse (LVN) license (required).
  • Current CPR certification (preferred).
  • Valid California Driver’s License with a clean driving record and insurability (required).
  • Live Scan clearance (required).
  • COVID-19 vaccination and booster (required).
