Duties & Responsibilities

Program Facilitation & Community Outreach

  • Coordinate and assist with core program operations, including patient enrollment, workshop facilitation, and daily activities.

  • Manage schedules for classes and workshops while coordinating coverage across various program sites.

  • Assist with outreach initiatives and referral programs, including conducting workshops and educational classes.

  • Support language access by providing translation assistance for monolingual patients.

  • Assist in coordinating partner meetings by gathering information, taking detailed notes, and managing follow-ups.

  • Review and maintain accurate patient information, operational documentation, records, and program reports.

  • Conduct regular program assessments, site visits, and community surveys.

  • Handle donation collections and maintain accurate financial reporting.

  • Order, receive, and maintain central inventory and program supplies.

  • Provide administrative support and coverage as Staff on Duty as needed.

  • Ensure strict adherence to all operational procedures, contract specifications, and regulatory standards.

  • Travel to other local service sites as needed within city limits.

Required Qualifications

Education & Experience

  • Education: Bachelor’s degree or equivalent years of professional work experience.

Language & Technical Capabilities

  • Language Skills: Required bilingual fluency in English/Spanish or English/Chinese.

  • Software Proficiency: Familiarity with Microsoft Office Suite and database management software.

  • Communication & Independence: Excellent verbal and written communication skills with the ability to follow detailed instructions and work independently.

  • Physical Requirements: Ability to stand for prolonged periods and occasionally lift up to 50 lbs.

Desired Qualifications

  • Senior Engagement: Minimum of one (1) year of experience working directly with older adults or senior citizens.

  • Outreach Experience: Minimum of one (1) year of experience and comfort engaging in community outreach efforts.

  • Specialized Certifications: Certification as a Diabetes Educator or Health Educator is a plus.

Location and Work Type

  • Work Type: Full-Time Position.

  • Work Setting: On-Site Senior Center Environment (with local travel required to other service sites as needed).

  • Schedule: Standard daytime hours, Monday through Friday.
