Working closely with an interdisciplinary team, you will educate patients and families, supervise therapy assistants and aides, and manage care outcomes across diverse clinical settings. What you will be doing:
Clinical Assessment & Patient Care

  • Perform initial and ongoing comprehensive assessments of patient needs, including OASIS assessments at designated time points, and evaluate overall care outcomes.

  • Participate in the development and implementation of patient plans of care in direct communication with attending physicians.

  • Deliver skilled restorative treatments to reduce pain and restore or maintain physical function, including gait training, ultrasound, electrical muscle stimulation, prosthetic training, balance/coordination exercises, and cardiovascular rehabilitation.

  • Perform range-of-motion (ROM) testing using a goniometer and utilize therapeutic modalities such as moist heat, portable whirlpools, or paraffin baths as prescribed.

  • Instruct, establish, and supervise individualized home exercise programs and provide home safety instruction to patients and families.

Team Collaboration & Supervision

  • Direct and supervise Physical Therapist Assistants, Certified Nursing Aides, and Home Health Aides in delivering patient care, performing supervisory visits as required.

  • Participate in regularly scheduled team meetings to coordinate care, exchange patient updates, problem-solve, and receive ongoing staff education.

  • Provide clinical instruction and educational guidance to patients, family members, and team members.

  • Accept direction from the Director of Professional Services, Patient Care Coordinator, or Rehabilitation Supervisor.

Quality, Compliance & Administration

  • Document all care accurately and submit clinical paperwork in a timely manner to maintain precise patient medical records.

  • Apply agency policies and procedures alongside federal, state, and certifying regulatory guidelines in all care delivery.

  • Participate in agency Quality Assessment and Performance Improvement (QAPI) initiatives as assigned.

  • Support organizational growth and resource efficiency by responsibly managing agency assets and educating the community about available services.

 

What you must have:
Education, Licensure & Certification

  • Education: Bachelor's or Master's degree in Physical Therapy from an accredited institution.

  • Licensure: Current, unencumbered Physical Therapist license in good standing with the state Board of practice.

  • Credentials: Valid driver's license and current automobile insurance.

Clinical Competencies & Skills

  • Specialized Knowledge: Solid understanding of physical therapy principles, concepts, and methodologies appropriate for terminally ill and complex patient care.

  • Patient Management: Knowledge and experience in bedside care, symptom management, crisis intervention, and family intervention.

  • Interpersonal & Decision-Making: Excellent communication, problem-solving, and decision-making skills, with a proven ability to build strong rapport with patients, families, and colleagues.


Nice to have:

  • Experience: One (1) year or more of work experience as a Physical Therapist within a multidisciplinary healthcare setting.
